302-5 Change of Student Name and Preferred Name

Policy: This policy is intended to address significant changes in name designation. Primary/legal names are used for student accounts, financial aid documents, payroll, official transcripts, diplomas, and on conduct and plagiarism records.

Change in Legal Name

Students may change legal names only if a legal document showing the new name/name change documentation is submitted to the Registrar along with a Student Account Information Update request. Examples of accepted documents include a Driver License, a Passport or a Permanent Resident Card, Marriage Certificate, a Court Order documenting a name change, or documents from the Social Security Administration.
